Rockford Center is seeking a dynamic and talented Assessment & Referral Counselor.
Rockford Center is Delawareâ€™s first private psychiatric facility and remains a leader in providing behavioral health care services and continues its commitment to providing quality health care.Â Rockford Center, a 138-bed acute psychiatric hospital continues to provide a full continuum of behavioral health services to geriatrics, adults, adolescents, and children.
The Assessment & Referral Counselor will assess and evaluate the needs of individuals in crisis, and Â appropriately triage callers based on information gathered, to the most appropriate service to meet their needs (this position is Monday - Friday 11am-7pm).
Key Responsibilities include:
Respond to telephone and face-to-face inquiries from individuals / agencies seeking mental health care, or information about mental health related services.Â Appropriately direct callers to the most relevant resources.
For individuals referred and presenting to this facility in a mental health crisis, conduct facility-approved clinical assessments utilizing appropriate documents and screening tools.
Counselors will be knowledgeable regarding department-specific performance improvement goals including, but not limited to, patient/family response times.
All calls and inquiries will be gathered using the facility-approved Intake Call form and all required data will be entered into the system as necessary.
Staff will be knowledgeable about services provided by other relevant mental health and other appropriate healthcare organizations within the community. This will include agencies such as social service agencies, crisis intervention services, shelters, state services etc. Employee will be knowledgeable about the local and regional systems of care, and demonstrate competency in accessing and coordinating information and care as appropriate.
To improve the patient experience by providing health care providers; patients and their families; and others with information, education, networking opportunities, and related resources focused on best practices.
