The MilitaryÂ & Veteran Liaison is responsible for providing indirect client care, outreach and case management services. Must serve as a member of the interdisciplinary team supporting the facilityâ€™s treatment program and philosophy as the primary liaison between the facility and outside agencies for discharge planning and continuum of care functions. Additionally, the Military & Veteran Liaison:
Serves as point of contact for referring Active Duty Providers and Veterans Affairs (VA) providers.
Available to accept referrals at any time, not limited to 8 hour work days.
Coordinates referrals with Intake and Clinicians to provide timely feedback to referral sources and schedule admissions.
Assist to coordinate discharge planning with Active Duty Providers and Commands and coordinate discharge planning with VA providers.
Assure that all reports on Active Duty admissions from the time of admission to the time of discharge are sent to Active Duty providers in a timely manner by fax or email.
Serve as a liaison between Active Duty Providers and the Chris Kyle Patriots Hospital team keeping leadership of the program up to date with all point of contact information, phone numbers, and email addresses.
Assure communication with VA referral sources, track patient authorization for treatment and referral on to the next level of VA care if private care not authorized.Develop improved communication with VA referral sources.
Assure an active role in the team to expand service lines, maintain/increase referral base, and supporting the overall business development strategies of the program.
Work with referrals from the Reserves and National Guard to facilitate activation of benefits, clarification of treatment needs, and admissions and discharge coordination of care.This is an underserved population, with current emphasis by Department of Defense to improve their access to Mental Health Care needs.
Work with the Chris Kyle Patriots Hospital treatment team to increase patient satisfaction by participating in treatment teams, meeting with patients to address unique needs and improve the treatment experience.
To improve the patient experience by providing health care providers; patients and their families; and others with information, education, networking opportunities, and related resources focused on best practices.
