ThedaCare, Inc., an integrated healthcare system in northeastern Wisconsin, seeks a board certified or board eligible Hospice & Palliative Medicine Medical Director to provide strategic leadership toward the development of a superior Hospice & Palliative Care Program. This position will collaborate with our system’s primary care physicians and specialists, as well as the interdisciplinary team to ensure patients and families receive high quality supportive care.
Serves as a key member of the hospice leadership team
Represents hospice and palliative care at the system level in collaboration with other physician leaders
Oversees clinical aspects of the hospice program and directs overall patient care policies
Responsible for admission/eligibility/recertification and discharge decisions
Performs home visits to hospice and palliative care patients
Provides consultation to hospice staff and community physicians on difficult cases
Influences and leads quality improvement cost containment activates
Provides in-service training for hospice staff and CEU offerings to community physicians
