This position is responsible for identifying, designing, implementing, and monitoring strategies to improve medication safety across the continuum at Banner Health. In addition, this position assists in coordinating and reviewing enterprise wide medication error reporting to design, educate, implement and monitor medication safety process improvement activities. This position works in close collaboration with patient safety, pharmacy and key stakeholders to coordinate medication safety activities. Essential Functions:- Provides programmatic oversight of the Medication Safety Program for all Banner Health entities. Develop strategies to minimize the potential for medication errors associated with the medication use process. Manage and oversee the development, planning, implementation and maintenance of the medication safety program.- Actively identify medication safety opportunities utilizing medication event reporting data, published best practice standards and analysis of internal and external medication use practices. - Coordinates, investigates, analyzes, and responds to medication safety issues, events, significant care issues or trends. Conducts surveillance, analysis and data reporting for medication safety, while complying with regulatory/accreditation requirements. - Assess/investigate medication safety related events. Facilitate root cause analysis, proactive risk assessments, variation reduction and identifies improvements related to medication safety and prevention of harm through the use of process improvement methodologies and tools.- Provide oversight in the development, revision, implementation and monitoring of medication safety policies/procedures, protocols and clinical practices. - Assist in the design and evaluation of automation and technology with the intent of improving medication safety.- Leverages lessons learned and communicate recommendations/required actions to avoid future exposure of harm to patients. Present data and reports, deficiencies identified during internal surveillance and monitoring, and the status of implementation to key stakeholders and identified committees. - Serves as a subject matter expert on internal and external medication safety matters. Collaborates with system and facility teams: care management teams, clinical discipline teams, facility leadership, risk management, and other groups as appropriate. Provides education and resources to address changes, identify gaps in compliance, and educate leaders and staff on medication safety requirements.

Advanced degree or equivalent working knowledge in the field.
- Strong background in medication safety.- Must have the ability to collect, analyze, and present data from a variety of sources to facilitate continuous improvements in medication safety initiatives.- Must possess demonstrated flexibility in responding to the needs of multiple constituencies with a service-oriented philosophy. Demonstrated ability to lead and facilitate interdisciplinary teams. Must also possess demonstrated skill in problem analysis, project management, conflict resolution and oral/written presentation. Requires ability to balance and manage multiple projects.
PGY2 or fellowship in medication safetyBoard certification by the Board of Pharmaceutical Specialties
