Work Hours/Shift: You Will Be Responsible For: - Creates, fosters and maintains professional relationships with staff and healthcare professionals
- Collaborates with supervisor and other team members, including medical staff, to identify/incorporate data collection from/into internal/external databases
- Abstracts designated information from medical records as specified by the appropriate abstraction guidelines, registry guidelines, and other technical specification manuals
- Maintains compliance with external regulatory agencies
- Plans and manages time appropriately to complete duties by assigned deadlines
- Coordinates and accurately completes assigned activities for data abstraction, including but not limited to accessing electronic medical and/or other records, abstracting prospective and/or retrospective data, requesting medical records, and entering data into data collection tools and/or databases
- Abstracts all patient records no later than 30 days post-discharge for retrospective reviews and during the visit for concurrent reviews
- Verifies accuracy and completeness of all record reviews
- Participates in inter-rater reliability reviews
- Maintains excellent computer skills in Excel, PowerPoint, Word and Access
- Assists with special projects, as assigned, to ensure the smooth operation of the program
- Effectively communicates relevant clinical information and opportunities for improvement and to supervisors and/or coworkers on a daily basis
- Distributes data to department leadership, executives and medical staff leaders in a useable format (e.g., Excel, PowerPoint)
- Provides support to other analysts
- Assists with other duties as assigned
- Required to meet department productivity standard(s)
- Works independently under indirect supervision
- Facilitates participation in clinical quality improvement utilizing lean tools and PDSA
CLINICAL: - Frequently uses clinical judgment independently to identify potential quality of care issues and/or opportunities to improve outcomes and/or processes
- Recommends processes improvement related to data abstraction, submission, reporting, and clinical care
- Maintains knowledge of mandated regulatory/publicly reported quality, safety, and performance improvement data requirements as specified by federal, state, payer and other agencies
- Reviews, assesses new data reporting requirements and alerts department leadership to new guidelines/criteria
- Facilitates use of data to develop proactive measures to improve clinical outcomes, patient safety and hospital processes by collaborating with individual members of the Medical Staff or hospital staff at each campus to concurrently meet evidence-based practice guidelines, individual departments to address improvement opportunities related to clinical practice, or participating in, facilitating or leading performance improvement teams or initiatives and works to achieve the performance goals set for improvement initiatives
- Communicates with other departments/disciplines, e.g. risk management, pharmacy, nursing, to report potential quality care/patient safety issues identified in the course of data abstraction, concurrently or retrospectively.
- Educates Medical Staff members and hospital staff on performance improvement philosophy, tools and methods
FINANCE: - Demonstrates appropriate utilization of resources necessary to obtain valid, reliable data for entry in the appropriate databases
- Considers and participates in other resource stewardship initiatives as appropriate for position
- Considers the financial implications and clinical outcomes when making recommendations and
developing program plans - Maintains compliance with departmental budget
GROWTH: - Attends quarterly teleconferences and seminars on topics related to assigned abstraction topics
- Seeks and participates in ongoing educational opportunities to update data abstraction knowledge and skills and integrates knowledge into daily practice
KNOWLEDGE AND SKILLS REQUIRED: - Working knowledge of medical terminology and anatomy
- Good organizational, filing, and writing skills
- Excellent computer skills, in navigating in Word, Excel, PowerPoint, and familiarity with Access
- Knowledge of medical record formats and use of a medical records system to locate data
- Ability to evaluate and correlate clinical information with appropriate definitions or specifications
- High degree of concentration to interpret medical records effectively and abstract data accurately
- Ability to work independently and efficiently from a remote environment
- Ability to provide self-direction in maintaining and improving data abstraction competence and cognitive aspects of medical/surgical nursing practice, current research, and quality measures
- English language proficient with effective writing, listening, speaking,/teaching communication skills
- Expert interpretation of raw data and comparative analysis
- Ability to work with data that is secure and confidential
Qualifications
What You Will Need: - Graduate of a School of Nursing
- Bachelor’s degree in a healthcare related field OR 10 years’ experience in clinical healthcare setting
- Minimum 3 years’ healthcare experience
- Current Florida Registered Nurse licensure
Job Summary: The Clinical Data Analyst (CDA) operates under the direction of the Operations Quality Improvement director/manager to review medical records, abstract pertinent elements, enter data into a specified registry and/or electronic data collection tool, identify trends in quality of care and patient safety, and implement the organizational quality management/ performance improvement program. The CDA is responsible for the management and maintenance of a comprehensive program of data collection and analysis of clinical care for internal and external benchmarking to improve patient outcomes and hospital processes. The CDA conducts concurrent and retrospective medical record reviews and data analysis to determine the need for care appropriateness reviews by the medical staff or committees. Prepares data for medical staff and hospital committees; participates on or facilitates performance improvement projects; participates in focused studies and detailed data collection with analysis; ensures complete, accurate and timely data is collected and maintained. Provides clinical analytic support to high priority cost and effectiveness initiatives throughout the system.
