This is an opportunity for a motivated APP to join the growing Palliative Care Department. The Outpatient Palliative Care APP evaluates and treats patients with serious illness, with focus on management of symptoms, care goals clarification, and emotional, practical and spiritual support. Depending on patient and family preference and capability, care may be provided in-person or virtually. The APP is a member of both the Palliative Care Division, and more specifically the interdisciplinary outpatient team (including administrative staff, nurses, PCT(s), physicians, social worker, mental health therapist). The APP participates as schedule allows in Division-wide meetings, and in regular outpatient team meetings. The APP manages their phone calls, EMR and email communications daily. The APP shares weekday phone/EMR back-up for colleagues, and is on-call for occasional daytime holiday and weekend answering service calls (less than 10 days yearly). The APP will participate in the education of learners (students and new employees), using clinical learning experiences, knowledge sharing and skill training. Must be NYS eligible for NP or PA certification. ACLS/BLS certifications required.
Requires a Master's Degree in Nursing or related field and an active New York State license in good standing. Applicants will have 5 years experience as an RN with one to three years of leadership experience; or equivalent combination of education and experience. The Nurse Practitioner influences the quality and delivery of care in various roles including consultant, researcher, mentor, and practitioner. This role may be unit, program, or service based and is involved in continuous quality improvement initiatives.
Completion of an AMA approved Physician Assistant program, licensure in good standing, and certified as a Registered Physician Assistant. Demonstrated clinical competence, superior verbal and written communication skills, interpersonal effectiveness and customer relation skills, computer competence and must have an active desire to learn. This role may be unit, program, or service based and is involved in continuous quality improvement initiatives.
