This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Learn more VA New Jersey Health Care System (VANJHCS) seeks to recruit a Health Promotion Disease Prevention (HPDP) Physician Program Manager to lead VA New Jersey Health Care System efforts to successfully meet the preventive needs of our Veterans. The Physician Program Manager will work with other clinical leaders to develop and deploy innovative strategies to address preventive care gaps including the newer challenges created by dramatic increases in "virtual" Primary Care in response to the COVID pandemic. The incumbent will lead and support Primary Care, Patient Care Services (Nursing), and Women's Health leadership as well as front line Patient Aligned Care Teams (PACT; Primary Care) in their prevention efforts. The incumbent works as an integral part of the Health Promotion and Disease Prevention Team that consists of two registered nurses and a social worker/health coach. The HPDP Physician Program Manager will review, analyze, and share prevention metrics across the health system and, with the other members of the Prevention Team is responsible for rolling out new preventive initiatives across the two medical centers and nine community-based outpatient clinics (CBOCs) that make up the VA New Jersey Health Care System. The Veterans Health Administration (VHA) is committed to raising the awareness of healthy behaviors and supporting Veterans and staff in their efforts to adopt healthy behaviors and reduce risky behaviors. VANJHCS is one of eighteen Whole Health Flagship sites in the country and as such has responsibility to transform care towards a Whole Health approach. The Health Promotion Disease Prevention Physician Program Manager will work closely with the services that support the Whole Health Flagship including the Center for Health and Wellness and Integrative Medicine leadership and staff. The HPDP Physician serves as a content area expert on changing health behaviors and with the Prevention Team is responsible for assessing and meeting the training needs of front-line staff with regard to initiatives related to behavior change including motivational interview training. Other key responsibilities include partnering with the PACT teams to spread and implement innovative strategies across PACT sites to meet the preventive needs of Veterans. The incumbent will participate in the many other HPDP activities including Tobacco Treatment, MOVE Weight Management, Diabetes, COPD, and CHF Whole Health Groups, Teach for Success Communication and Motivational Training, Coaching PACT teams, and integrating Whole Health and innovative strategies into meeting prevention needs of Veterans across VANJHCS. The VA New Jersey Ambulatory Care Service is affiliated with Rutgers New Jersey Medical School and the HPDP Physician includes the opportunity for an academic faculty appointment and supervision of trainees including Preventive Medicine residents.
