The Clinical Pharmacy Specialist (CPS) for Ambulatory Care is a licensed pharmacist with a Doctor of Pharmacy degree who has completed an American Society of Health Systems Pharmacists accredited Clinical Pharmacy Practice Residency, or other equivalent experience. The incumbent is responsible for Ambulatory Care clinical pharmacy activities and consultations in Patient Aligned Care Teams (PACT), Womens Health, Specialty Care, and the Community Based Outpatient Clinic in Evanston, Illinois.
