An opportunity exists for a full-time Primary Care Physician located at the Hoffman Estates Community Based Outpatient Clinic of the Edward Hines Jr., IL VA Medical Center The General Medicine Service is the largest patient care service within a large, tertiary care complex, VA Medical Center. The Community Based Outpatient Clinics (CBOCs) are a part of a large, complex, VA Medical Center in the delivery of Primary Care. The Edward Hines Junior VA Hospital is seeking applications for a full time Primary Care physician to be located at its Hoffman Estates Community Based Outpatient Clinic (CBOC). CBOC Primary Care manages the primary care needs for roughly half of all patients vested in primary care at Hines. Over 30 PACT provider FTEE are deployed over 6 CBOC locations to provide care to over 20,000 unique veterans. Five of the six CBOC locations are leased properties of approximately 10,000 square feet in size, with one CBOC being an owned property of 60,000 square feet, with 40,000 square feet being clinical space. While growth in primary care unique veterans has been modest at best in our main campus location for the last several fiscal years (with the exception of female and OIF/OEF veteran patients), growth in unique veterans across 4 of the 6 CBOCs has been sustained for several fiscal years. In essence, over time, CBOC care is only expected to become larger, more complex, and more essential to maintaining VERA funding and care delivery to unique veterans that use the Edward Hines, Jr. VA Hospital healthcare system. Pay: Competitive salary, annual performance bonus, regular salary increases Paid Time Off: 49-54 days of annual paid time offer per year (26 days of annual leave, 13 days of sick leave, 10 paid Federal holidays per year and possible 5 day paid absence for CME) Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement) Licensure: 1 full and unrestricted license from any US State or territory CME: Possible $1,000 per year reimbursement Malpractice: Free liability protection with tail coverage provided Contract: No Physician Employment Contract and no significant restriction on moonlighting The Primary Care Provider will provide primary care, to include Women's health care, for a panel of approximately 1200 patients at the Hoffman Estates Community Based Outpatient Clinic (CBOC), one of six CBOCs for Hines VA hospital (others located in Bourbonnais, Joliet, North Aurora, Oak Lawn, and Peru, Illinois). Incumbent works collaboratively with local CBOC staff and projects involving CBOC staff or patients. Incumbent will participate and maintain clinical and educational activities within Primary Care/General Medicine at Hines to stay grounded and relevant in the role. Women's Health Care updated training will be provided as needed. Must be Board Certified or Board Eligible in Internal Medicine or Family Medicine Responsible for all functions which fall under the scope of care of Primary Care. Responsible for all functions which fall under the scope of care of a Primary Care Provider. These include predominantly clinical and administrative management, either directly or through delegated authority of the following: Initial patient evaluation Assists individuals/families to achieve optimal levels of health through collaboration with medical and health care professionals Facilitates the delivery of primary, acute and chronic health care Exercises medical decision-making and provides a broad range of diagnostic and therapeutic services for a specific group of patients Exhibits leadership in initiating changes in patient care programs based on current concepts and findings from research and/or studies Involved in direct patient care, patient and family teaching, staff education and multidisciplinary consultations and collaborations Participates in education of PA students, NP, Residents, Medical Students, interns if needed Supervise mid-level practitioners as assigned Consistently demonstrates expertise in diagnosing and treating seriously ill multi-symptomatic patients Serves as a resource person for standards development in health care in the area of quality improvement concerns Management of acute and chronic medical conditions in an outpatient setting Consult other services as needed based on strong evidence based guidelines Work Schedule: Monday - Friday, 8:00 AM - 4:30 PM
