Conduct patient visits in the home including medication reconciliation
Assess existing conditions; diagnose and formulate plan of care
Write referrals, renew medications, order diagnostics and manage results.
Complete POLST (Physician Order for Life-Sustaining Treatment) for patients as necessary.
Advanced directive and advanced life care planning activities are a priority in this position.
Work Schedule: 4 days a week / 7AM-6PM
Avg Daily Patient Census: 6/day Senior population and some younger patients who are medically disabled and on Medicare supplemental program.
Experience with treating patients with multiple co-morbidities
Posses advance critical thinking skills
Strong communication skills
Current Nevada R.N. and N.P. license
Certificate of completion from an accredited Nurse Practitioner Program
Current Nevada DEA certificate required prior to start date
At least one year experience practicing as an N.P. New grads with a history of seeing patients in their homes or with Geriatrics will be considered
Must have valid driver’s license and insurance
Must be comfortable conducting home visits with high-risk patients
Two years of experience in Primary Care or Hospice
