The role you’ll contribute:
The Quality/Performance Improvement (PI) Manager is responsible for performing activities of the performance improvement program and related functions of the home health department. Related functions include but are not limited to staff development, education, coding, working reports related to billing and certification review. The Quality PI Manager operates independently under the general supervision of the Agency Administrator.
Serves as a resource person to staff regarding agency operation and regulatory agency/intermediary requirements.
Optimizes the smooth transition of care from referral source to home.
Fields initial admission calls from clinician in patient home to ensure admission criteria/payer eligibility is met and patient is involved in establishing the care plan.
Works with admitting clinician to establish initial care plan for all disciplines following utilization guidelines based on findings from the initial patient assessment.
Communicates effectively with all team members to ensure care is appropriately set up and managed for the patient.
Coordinates staff education and clinical assessment, inclusive of new hire orientation, in-service education to the interdisciplinary team and annual competencies, addressing identified needs of staff; infection control, performance issues.
Supervises and evaluates staff reporting to this position.
Coordinates home health performance improvement activities. Ensures that patient outcomes are monitored and improved through appropriate safety/preventive measures.
Facilitates and reports on quarterly record review and analysis of potentially avoidable events.
Ensure that all admission records are reviewed for accuracy including certification and OASIS congruency and coding; assists in working batch error reports, bill hold and other billing reports, checks orders for completeness prior to mailing.
Assures that bill holds and unbilled reports are addressed and coaches staff to meet requirements.
Ensures that opportunities to improve patient care are taken and identified problems are resolved.
Facilitates the evaluation and annual revision of the objectives, scope, organization, and the effectiveness of the performance improvement program. To include collection and use of comparative data; assists with the implementation and monitoring of all aspects of the annual performance improvement program.
Participates in staff meetings, department meetings, in services and other related activities.
Attends educational and developmental seminars and meetings, as deemed appropriate.
Maintains membership of and participates in applicable home care-related and professional organizations.
The expertise and experiences you’ll need to succeed:
Associate degree in nursing.
Minimum 3 years home health experience as a Registered Nurse.
A clear/active Florida Registered Nurse (RN) license.
Basic Life Support certification .
Maintain a current, valid Florida Driver's License in good standing with a safe driving record and a reliable vehicle.
