WHO WE ARE LOOKING FOR:

Franciscan Physician Network is seeking a full-time Family Medicine Nurse Practitioner to join our rural healthcare practices at Franciscan Health Brook Health Center in Brook, Indiana. This provider would join a growing, thriving practice that provides a collegial environment and strong mentorship opportunities.

WHAT WE NEED:

Ideal candidate will have a strong focus on clinical excellence and patient satisfaction. Candidate will display exceptional interpersonal skills and a desire to develop strong ties with the community and build long-lasting relationships with patients. Physician will need to work collaboratively to coordinate and deliver the highest quality of patient care.

WHAT YOU CAN EXPECT:

• Clinic Hours: Monday - Thursday, 7:00 a.m. - 4:30 p.m.
• Flexible full-time schedule; No weekends or holidays
• Will treat patients of all ages
• Telehealth offered
• Common Diagnoses:
  • Child and adult wellness visits
  • Back pain
  • Endocrinology issues
  • Hypertension
  • Heart failure

The town of Brook, Indiana is located in beautiful Newton County. Located 1.5 hours northwest of Indianapolis, Brook has both a rural and suburban feel, with a large heart and personality. Brook is next to Rensselaer, Indiana, which offers affordable cost of living with excellent school options including private and public schools.

Qualifications:

• Master's Degree - Required
• Nurse Practitioner (NP) or Nationally Recognized Certification - Required
• Basic Life Support Program (BLS) - American Heart Association - Required
