**This will be a hybrid position working from home and onsite
Under the direction of the Director of Medical Management; this extremely organized, self-motivated and critical thinking individual, effectively manages, the day to day operations of reference programs included but not limited to:
Whole Life, Medication Refill, Dietary/Health Education and Behavioral Health-operations support
Management activities of these programs and initiatives align thru Memorial Care’s strategic goals and commitment to quality care.
Gain cross leadership support, working with operations team to drive to optimal outcomes.
Assurance that these programs and initiatives are focused and aligned to improving operational and program efficiencies and effectiveness consistent with strategic vision, scope and mission of MemorialCare.
Participates in strategic planning and provides leadership for program development.
Evaluates the impact that systems improvements have on the organization’s ability to provide quality care.
Oversees staff conducting clinical and administrative support and reporting for the program including; recruitment, coaching, schedules, payroll, triage, annual reviews and engagement surveys, reports and other identified activities.
Ensure workflow is in alignment with all required items to support aligned programs.
Collect and summarize quality improvement data and report findings and outcomes to leadership in a timely manner, using a standardized format.
Collaborate with and represent Memorial Care Foundation and providers professionally and via internal and external stakeholders involved in the assigned programs and initiatives.
Develops and implements policies and procedures for the identification, collection, analysis and reporting of program related data and information.
Generates weekly and monthly reports for assigned programs including but not limited to utilization, performance (COC, patient satisfaction, discharge, etc.), quality, and budget.
Provides Project Management Support to department for expansion of services, programs, and infrastructure as needed.
Coordinates dashboards, and performances improvement initiatives.
Additional management activities to assigned programs as stated above and Behavioral Health operations with responsibilities including but not limited to:
Daily communication support of programs and services
Maintenance of and support to marketing pieces needed for programs
Oversight and troubleshooting of essential program functions
Scheduling and coordinating meetings, minutes, follow up
Maintaining medical and non-medical supplies, phones, lab coats and other materials necessary to functioning of programs
As needed support for meetings including scheduling, set up, telecommunications, follow up, agendas and minutes
Consistently and reliably communicates with Director of Medical Management to ensure that all functions, operations, projects and data reporting of assigned programs are functioning at highest level.
Effectively prioritize and multitask workload in a fast paced and changing healthcare setting
Always Maintains confidentiality and professionalism .
To improve the patient experience by providing health care providers; patients and their families; and others with information, education, networking opportunities, and related resources focused on best practices.
